Remarks:
1. Once you come back to Thailand, you must report yourself within 24-hours. Please click on
the “24-hour Notification” bottom (number 7) and fill out the form.
2. To ask permission to leave Thailand, please fill out the “Request Form for Traveling Outside
of Thailand”. Please click the “Request to Travel” (number 8) and fill out the form.
